Creative Health Quality Framework – Survey

Help the Culture, Health and Wellbeing Alliance understand how you have been using the Creative Health Quality Framework. We’re keen to hear from everyone who is involved in delivering, commissioning, funding or supporting Creative Health work.

There are two parts to the survey:

Part 1 is about your experience using the Creative Health Quality Framework

Part 2 is an Equality, Diversity and Inclusion form

The whole survey should take no more than 10 – 12 minutes to complete. All questions are optional and your responses will remain anonymous. The link to Part 2 of the survey is provided once you have submitted Part 1.

We will use your feedback to inform the next phase of the framework’s development and assist us in applying for additional funding to make revisions and create additional resources. We encourage you to be as open and as detailed as possible.

The deadline for completing the survey is 12pm noon on Friday 26th April 2024.
