How does social prescribing support people with long-term conditions?

Social prescribing is increasingly established as an approach within primary care that supports people struggling with loneliness, isolation or complex social issues that affect their health and wellbeing. But a wide range of projects also demonstrate the crucial role it can play in the prevention and management of conditions like diabetes, cardiovascular problems, dementia and cancer, and the benefits it can bring to people living with multiple conditions.
